4 vital steps RUSH took to reduce readmission rates

These are RUSH’s top four learnings from a readmissions-reduction initiative led by Dr. Brian Stein, chief quality officer, and Tom Webb, former associate vice president of quality analytics.

  1. Have the right data. RUSH uses the CDB to drive benchmarking and performance improvement.
  2. Understand your patient population. Assess commonalities for frequently readmitted patients and social determinants of health.
  3. Connect the dots across multiple services. Connect services to manage patient care, remove barriers and ensure care continuity so no patients slip through the cracks.
  4. Gain leadership buy-in. Make reducing readmissions an organizational priority at every level cascading quality goals from top.
