Patients undergoing surgical or invasive procedures are more vulnerable to communication errors occurring during the numerous transitions in care from one provider or clinical setting to another. Inadequate preoperative assessment and optimization for surgery as well as lack of patient engagement and care coordination contribute to surgical cancellations and delays, poor patient experience, complications, extended hospital stays and mortality.
Based on a Vizient® Patient Safety Organization (PSO) analysis of perioperative care coordination issues identified in safety reports, and with guidance from an expert advisory team, the Vizient PSO developed a leading practice document around the following areas:
