2019 Performance Improvement Collaborative Series
Join us on April 24 when we feature Vizient’s Performance Improvement Collaborative to reduce hospital readmissions. Readmissions are potentially detrimental to patients with increased health risks, unfavorable outcomes and decreased satisfaction. They are detrimental to hospitals, bringing reduced reimbursement and increased costs, an estimated $26 billion per year — of which $17 billion is avoidable. We’ll learn how participants improved processes in discharge planning, coordination with post-acute sites, transitioning patients to those sites and automating care coordination.
We’ll also hear from leaders at NYU Langone Health who’ll describe how their team forged partnerships with post-acute care sites and the community to drive down readmissions to their organization.
